BOOK A VACCINATION CLINIC APPOINTMENT 

  • Offered Vaccines: 2025-2026 Flu, COVID
  • Please sign up for an appointment using the QR code or call (707) 528-3730.
  • Complete and bring printed copy of consent form to clinic.
  • Bring a copy of medical and prescription insurance information.
  • Kaiser / Medi-Cal is not accepted - SHS will have FREE FLU SHOTS for anyone without insurance.
  • Wear a short-sleeve shirt to your appointment, if possible.
