ELIGIBILITY
All students enrolled in any credit course in the Sonoma County Junior College District can access services funded by the Student Health Fee. This includes free professional visits and certain supplies. However, students must cover the cost of care at other healthcare facilities.
Non-credit students have access to our Online Services, Education and Outreach Resources, and referral resources.

Our Legal Duty
We are required by law to protect the privacy of your information, provide this notice about our information practices, follow the information practices that are described in this notice, and obtain your acknowledgment of receipt of this notice.

Individual Rights
In most cases, you have the right to look at or get a copy of health information about you that we use to make decisions about your care. You also have the right to receive a list of instances where we have disclosed health information about you for reasons other than treatment, payment or related administrative purposes and other than when you explicitly authorized it. If you believe that information in your record is incorrect or if important information is missing, you have the right to request that we correct the existing information or add the missing information.

Uses and Disclosures of Health Information
Student Health Services staff include District employed support staff and medical clinicians, District contracted physicians, District contracted mental health providers, and community health clinic providers with whom we have a Memorandum of Understanding in place to offer clinical services on site. In the interest of providing continuity of care, information may be shared among these providers and with other providers to whom you are referred, only to the extent that it ensures appropriate treatment. We may use health information about you among these providers for administrative purposes, to evaluate the quality of care that you receive or to obtain payment for treatment received and not covered by your student health fee. Information may be shared by paper mail, fax, or other confidential methods. At no time is this information shared with other college administrators, faculty or staff without your specific written authorization.
We may use, disclose or allow access to identifiable health information about you without your authorization for several reasons. Subject to certain requirements, we may give out health information without your authorization between the following: District employees of Student Health Services and contracted providers within Student Health Services; medical record database management entities with whom Student Health Services holds a contract; for training, public health; or auditing purposes; for research studies; and for emergencies. We provide information when otherwise required by law, such as for law enforcement in specific circumstances. We may use or disclose your protected health information as necessary to contact you or remind you of your appointment. In any other situation, we will ask for your written< authorization before using or disclosing any identifiable health information about you. If you choose to sign an authorization to disclose information, you can later revoke that authorization by writing a revocation statement to the Director of Student Health Services (address below) to stop any future uses and disclosures.
We may change our policies at any time. Before we make a significant change in our policies, we will change our notice and post the new notice in the waiting area. You can also request a copy of our notice at any time. For more information about our privacy practices, contact the person listed below.

HEALTH INSURANCE AND ENROLLMENT
Apply During Open Enrollment
Anyone can generally buy marketplace health insurance only during the annual Open Enrollment period.
- Open Enrollment starts November 1 and goes through January 15 each year.
Applying Outside of Open Enrollment
If you missed Open Enrollment (November 1–January 15), you may still be able to get health coverage if you qualify for a Special Enrollment Period, due to a Life Event.
Qualifying Life Events include:
- Getting married or divorced.
- Having a child or adopting.
- Losing coverage because you turned 26, left your job, or your job benefits were reduced.

Medi-Cal
People who qualify for Medi-Cal can apply for health insurance at any time.

Assistance with Enrollment
Make an appointment to speak with a Certified Enrollment Counselor at Student Health Services.
With this provider, you can:
- Discuss your insurance needs.
- Apply for Medi-Cal or private insurance coverage.
- Receive assistance if you’ve applied but have questions about coverage or when your coverage starts.
- Receive assistance in locating a local primary care provider and getting established as a regular member.
- Enroll in Cal-Fresh.
PATIENTS RIGHTS AND RESPONSIBILITIES
As a patient you have the right to:
- Information on the privacy and confidentiality practices with regard to your treatment and medical records.
- Respectful, courteous, and considerate care.
- An explanation of services provided, including any applicable fees, and to be made aware of provisions for after hours and emergency care.
- Complete information concerning your evaluation, diagnosis, treatment and prognosis.
- Review (or request a copy of) your medical records in the company of a clinician and request an accounting of disclosures or an amendment of information contained in your record.
- An explanation of the effectiveness, possible risks and side effects of treatment or alternate methods of treatment.
- Choose a clinician or to change clinicians if desired when a qualified alternate clinician is available.
- Know who is providing care to you and what credentials they hold.
- Refuse treatment, or to ask for a second opinion, or an alternative course of treatment,and to be informed of the medical consequences of your actions.
- Information on personal responsibilities involved in seeking medical treatment and maintaining health and well being during and after treatment.
- Bring any suggestions or dissatisfaction to the attention of the attending clinician, the Director of Student Health Services or the Vice President of Student Services without intimidation, discrimination or retaliatory action when accessing future care.
As a patient it is your responsibility to:
- Present accurate identifying information.
- Present details of illness or complaint in a direct and straightforward manner.
- Cooperate respectfully with all persons involved in the health care process.
- Arrive to appointments on time, canceling only when absolutely necessary, and far enough in advance so that other patients might utilize that time.
- Follow the treatment plan provided by the clinician, asking for clarification whenever necessary, and/or express concerns regarding your ability to follow the treatment plan.
- Accept outcomes of choosing not to accept treatment or follow a clinician’s recommended treatment plan.
- Understand the scope of services provided here, any associated costs, and the necessity of medical insurance.
- Provide both positive and negative feedback to the clinician responsible for your care and/or to the Director of Student Health Services.

What is the Student Health Fee?
The Sonoma County Junior College District has established Student Health Services as an integral part of the college community. The intent of the Board of Trustees in providing Student Health Services at SRJC is to monitor and improve the health and safety of the college community and to support student retention and academic success by addressing the student health needs.
California Education Code (76355) allows a Health Fee to be charged to students, and at Santa Rosa Junior College, this is applied to all credit-enrolled students, regardless of class location. The maximum amount of the Health Fee is outlined by the Education Code, with $1 cost of living increases allowed when inflationary indexes in California warrant it. Additionally, the Education Code language includes regulations on what services may be provided with Health Fee dollars.
What does the Health Fee pay for?
The Health Fee is the primary source of funding for programs offered by the Student Health Services department. This includes the salaries of professional staff in the health centers established on the Santa Rosa and Petaluma campuses. The staff provides individual healthcare services, such as nurse practitioner and physician visits, mental health crisis counseling, as well as health promotion in classrooms, online, and at campus events. Staff also provides support services towards improving the health of the entire college community through public health and risk management interventions, such as emergency preparedness, Incident Report tracking/analysis, and trainings. The Health Fee also pays for some medical supplies, such as onsite lab work and antibiotics, to assist in the control of communicable diseases.

Why must all students pay the Health Fee?
The public health funding model, as designed by the Education Code and adopted by the Board of Trustees, invests Health Fee dollars towards sustainable and effective health services within the entire college community.
When any individual student with a communicable disease is being treated by Student Health Services, this keeps all students, staff and faculty on our campuses healthier by reducing exposure to infections.
When Student Psychological Services offers drop-in Crisis Intervention for distressed students, this helps keep disruptive situations out of the classrooms and alleviates anxiety felt by their peers and faculty, by connecting them with the help they need.
Online resources are provided, which all students may access, including a broad range of informational and interactive sites, Sonoma County healthcare resources, on-line mental health screenings, and Student Health 101, an online health magazine. This contributes to a healthier educational environment for everyone.
Health Fee Waivers
The District currently allows waivers for:
- Students currently enrolled in Study Abroad programs.
- Incarcerated students.
- Students that rely exclusively upon prayer for healing (form from Admissions & Records available here).
- Students in qualified apprenticeship programs.
- When dual-enrollment CCAP courses are physically at high schools.
These waivers are outlined in District Policy 5.4P.
Who may access Student Health Services, and what is available?
Any student enrolled in a credit course may access free healthcare visits with the professional staff. Whereas many of the students coming into the health centers for this service are economically challenged and do not have health insurance, just as many students DO have health insurance, finding the location convenient, and without costly co-payments for healthcare visits in the community.
These services include:
- Over the counter items, such as ibuprofen, condoms, and band aids
- Visits with nurse practitioners, physicians, and mental health providers
- Treatment for acute injuries and illnesses
- Individual, couples, family, and group therapy
- Crisis intervention drop-in services
- Emergency contraception and pregnancy testing
- Assistance meeting health requirements associated with academic programs
- Information on MediCal enrollment, and health insurance options
- Assistance with Student Accident Insurance claims
- Community referrals and resource-finding
- Immunizations* (provided at cost)
How does a student access services?
Walk-in appointments for medical services are accommodated as possible, though appointments are preferred. For mental health appointments, students simply come to our office in Bertolini (Santa Rosa) or Call (Petaluma), complete paperwork, and have an initial assessment. Immediate appointments are typically available during weekday walk-in hours, although students can be seen at other times if in crisis or if not available during regular drop-in hour (by arrangement).
