If you have the “Partnership” Plan (typical for those in Sonoma County):

• Go to: https://www.carelonbehavioralhealth.com/
• This brings you to a page called Carelon Behavioral Health, formerly Beacon Health Options.
• On a desktop computer, go to the top bar. On a phone click on the three little lines in upper right.
• Click on “Find a Provider”
• On the next page, where it asks you to enter the first three letters of your health plan, enter PAR.
• Then, click on “Partnership Health Plan of California”
• Click “Submit”
• It may or may not ask you to put in your address.
• You should now have a long list of providers. On the desktop version, there are a number of filter options on the left should you want to filter by distance, or tele-health only, or many other options. On the phone search, the filter options are at the very top.
• You are now ready to call people on the list. Note that you are likely to get their voice mail. We recommend calling multiple people - as many as 10, as some will not call you back due call volume, and others may tell you they have no openings. But if you call a bunch of people, you will find someone, even if it takes making a bunch of phone calls.
